Hy-Vee Pharmacy in Mount Pleasant has been cited and fined by the Iowa Board of Pharmacy following an investigation into a medication dispensing error.
The Board approved a combined statement of charges, settlement agreement, and final order involving the pharmacy after determining violations occurred in November of 2025.
According to the order, the pharmacy filled a prescription for a five-year-old child for the antibiotic Cefdinir. The prescription called for a dose of 125 milligrams per five milliliters, with the child receiving five milliliters twice daily.
The pharmacy did not have enough of that medication strength available and instead filled the prescription using a stronger concentration. The directions were adjusted to two and a half milliliters twice daily, but the pharmacy support person who provided the medication to the child’s father did not ensure counseling by a pharmacist, as required for a new prescription and a change in dosing instructions.
The error resulted in the child receiving twice the intended dose of the antibiotic for five doses before the mistake was discovered.
During the investigation, the Board also found the pharmacy support person involved had been working in the pharmacy without the required registration since March of 2025. The individual has since applied for and received an active registration.
The Iowa Board of Pharmacy charged Hy-Vee Pharmacy in with failing to provide required pharmacist counseling and employing an unregistered pharmacy support person.
As part of the settlement, Hy-Vee Pharmacy was issued a citation and warning and must pay a $1,000 civil penalty. The pharmacy is also required to comply with all state and federal pharmacy laws and regulations.
The Board says failure to comply with pharmacy rules in the future could result in additional disciplinary action. The order is now part of the pharmacy’s permanent licensure record and has been reported to the National Association of Boards of Pharmacy.
