The Iowa Board of Pharmacy has placed PerfectRx in Washington on probation and issued a $5,000 fine following an investigation into the pharmacy’s handling of compounded medications.
The Board approved a combined statement of charges, settlement agreement, and final order involving PerfectRx, which holds an Iowa pharmacy license that remains active through the end of 2026.
The investigation began after the Board received two complaints against the pharmacy. According to the order, PerfectRx dispensed patient-specific prescriptions using compounded preparations made by ProRx, an outsourcing facility that was not licensed in Iowa.
The Board found PerfectRx began obtaining and dispensing compounded medications from outsourcing facilities in the spring of 2025, before an Iowa rule change took effect allowing that activity.
The investigation found approximately 1,210 prescriptions for compounded GLP-1 medications were dispensed to Iowa patients between March 17th and November 6th of 2025. The pharmacy also reported more than 2,800 vials of ProRx GLP-1 products were removed from the facility and returned to an affiliated PerfectRx location in Texas in November.
The Board also found PerfectRx continued dispensing compounded GLP-1 medications after those drugs were no longer listed on the FDA drug shortage database and after the FDA’s enforcement discretion period had ended. The Board said the pharmacy did not provide a clinically significant justification for continuing those prescriptions beyond providers indicating patients needed a “custom dose.”
PerfectRx stated the issues resulted from errors made by the pharmacist in charge of the facility at the time, who has since been removed from that position. The pharmacy also said it has implemented new policies and procedures to address the concerns.
