An Ottumwa physician has agreed to surrender his Iowa medical license following allegations that he failed to comply with an order from state regulators.
According to the Iowa Board of Medicine, Dr. Akhtar Ashraf, who was first licensed to practice medicine in Iowa in 1990, has been under investigation since 2023. Ashraf’s practice includes diagnostic radiology.
Board records show that in June 2025, Ashraf was issued a confidential order requiring him to undergo a clinical competency evaluation within 30 days. The board has not publicly disclosed why that evaluation was ordered.
Regulators recently alleged that Ashraf failed to comply with the order or schedule the required evaluation. According to the board, the 80-year-old physician has since informed officials that he is retiring from the practice of medicine in Iowa and wishes to voluntarily surrender his medical license.
The Iowa Board of Medicine is expected to consider the license surrender as part of its disciplinary proceedings.
